Greenwood (Traditional) Day Camps
Per Massachusetts Board of Health, 105 CMR: 430, updated physical (no older than 18 months prior to camp date) and immunization records are REQUIRED at the time of registration and inspected by the Longmeadow Board of Health.
Special note for online registrations: Registering online places an additional burden on you, the parent, to be solely responsible for getting these forms to us immediately. These health forms must be received in our office at the Community House by no later than 5pm, the Wednesday prior to the camp's begin date. Failure to provide health forms by this time will result in withdrawal from the camp, a $25 cancellation fee, and possible re-enrollment late fee of $10.
